Elderly Health Assessment Clinic

When our patients reach the age of 80 they are sent a questionnaire and may be invited to come in for a check-up thereafter. The aim is health promotion and prevention, enabling older people to stay healthy and to enjoy life to its fullest potential.

A holistic approach is taken. General health issues are discussed, blood pressure and urine are checked and patients referred to their GP as appropriate. They will discuss any worries or problems the patient may have, either medical or personal. If they can’t answer questions, they will find someone who can. Referrals can also be made to occupational therapy, home care, care managers and many other agencies. If at any time patients are unable to get to the clinic they will be visited at home
