If you think you may require vaccinations, you will need to complete our Travel Risk Assessment form The complete every section as fully as possible.
Please ensure this is returned to the practice at least 8 weeks before your travel date. On return of the form our Receptionists will book an appointment with the Nurse to discuss your travel requirements.
At busy times we may direct you to other travel clinic providers to ensure you receive the travel advice and any medication in time for your trip.
