If you think you may require vaccinations, you will need to complete our Travel Risk Assessment form The complete every section as fully as possible.
Please ensure this is returned to the practice at least 8 weeks before your travel date. The Nurse will review the form and contact you to book an appointment if you require any vaccinations.
At busy times we may direct you to other travel clinic providers to ensure you receive the travel advice and any medication in time for your trip.
